Medical Census

In order to analyze our Departments health benefit programs, we need to provide our insurance broker with certain information about you and your dependents. While we may or may not have your information, we likely do not have such information as birthdays for employees and your family or changes within your family unit. Since the collection of this kind of detailed information can be cumbersome and time consuming, we have decided to utilize an online tool called PRO Census to help expedite the process for you and us.

PLEASE NOTE: ALL Deputies, Sergeants and Captains regardless if you are an FOP member or not needs to fill out the ‘medical census” form as the FOP is the registered Bargaining Agent we negotiate for your health benefits during the collective bargaining process and its vital that we have this information on all employees of the bargaining unit.

By clicking on the link below you will be taken to the PRO Census site. There you can quickly and securely provide the information needed. Once completed, this information will be accessible by our insurance broker and other third parties that are necessary in the analysis of our health benefits program.
